Cystitis in general
Cystitis lead to a persistent urge to urinate, hand in hand with a burning sensation when urinating.
Mostly there is frequent urination with small amounts of urine, sometimes blood in the urine, or cloudy or strong-smelling urine. Furthermore pains, cramps and discomfort in the pelvic area and the lower belly.
If there are bacteria involved, it is called a bacterial cyctitis and the family doctor will prescribe antibiotics. Sadly enough there are many forms of cystitis, without bacteria as a cause, and these are not responsive to antibiotics!
Nonbacterial cystitis is a catch-all term that for many medical disorders, including infectious and noninfectious cystitis, as well as painful bladder syndrome/interstitial cystitis (PBS/IC), of course all without bacteria as cause.

Noninfectious nonbacterial cystitis
Noninfectious nonbacterial cystitis includes the following forms of the disease: viruses, fungal organisms and such. Next to these forms of cystitis there are inflammations of the bladder caused by radiation as cancer treatment, autoimmune disbalances and hypersensitivity.
In case of repetitive forms of cystitis the treating physician or urologist will have to rule out many causes such as cancer and all kinds of inflammations by repeated cultures and various urologic procedures, such as cystoscopy, bladder biopsies, functional bladder tests, and tests for the immune system.
Interstitial cystitis (IC)/bladder pain syndrome (BPS): a chronic bladder health issue.
If the causes of infection and related causes are excluded by the urologist, sadly enough there are many patients left with no clear diagnoses. These unfortunate patients suffer from a bladder pain syndrome caused by derangements of the immune system, resulting in hyperactive inflammation cells, such as the mast cells. In biopsies of the bladder one can find higher numbers of these inflammation cells, and the cells have higher activity. Especially the mast cell is a nasty fellow. On the picture you can see how in the bladder a vicious circle emerges based on the over activity of inflammatory cells.
